Case Manager

This is a fully remote position, open to applicants in New York.

📋 Description

• Evaluate the patient's comprehensive range of immediate and long-term needs by reviewing their social and medical history.

• Create a tailored care plan based on clinical assessments from both the patient and care providers.

• Regularly assess and adjust the care plan within the framework of case management practices.

• Foster professional relationships with all members of the interdisciplinary team.

• Serve as an advocate for the patient.

⛳️ Requirements

• Must be a Registered Nurse (RN) holding an active, current, unrestricted license in a Nurse Licensure Compact (NLC) state.

• A Bachelor’s degree is preferred.

• Case Management Certification (CCM) is mandatory.

• A minimum of three (3) years of clinical RN experience in a med/surg or ambulatory care setting is essential.

• Clinical experience in at least one of the following areas is preferred: Behavioral Health, Transplant, Neonatal, or Oncology.

• Previous experience in managed care is advantageous.

• Strong problem-solving skills along with effective time management abilities are required.

• Exceptional written, verbal, and interpersonal communication skills are necessary.
