• Serve as a member of a multidisciplinary team aimed at delivering services from a holistic and values-driven approach.
• Collaborate with program participants, clinical and medical staff, and other stakeholders to assist in the formulation and review of recovery plans.
• Build and nurture relationships with community organizations to help program participants access all available resources.
• Lead team efforts for the creation and annual review of the Individual Service Plan (ISP).
• Continuously monitor the ISP, document outcomes, and take action as needed.
• Prepare and evaluate Critical Incident Reports (CIR) following established protocols.
• Engage in and oversee reviews conducted by the Human Rights Committee (HRC) and Behavioral Intervention Committee (BIC) when appropriate.
• Emphasize Personal Outcome measures.
• Possess knowledge and understanding of the Charting the Life Course framework and Person-Centered Thinking/Planning.
• Assess the needs of individuals on your caseload to ensure their success within the community.
• Connect individuals on your caseload to necessary and requested programs and support services.
• Complete all required service documentation, including entering data, reports, and plans into Therapy.
• A Bachelor’s degree and one year of relevant experience or an equivalent combination of education and experience.
• Proficient intermediate computer skills.
• Ability to establish a HIPAA-compliant home office.
• A valid driver’s license.
