This is a fully remote position, open to applicants in United States.
• Overseeing and managing home-bound clients/members.
• Monitoring health care plans for members enrolled in the EDWP Elderly and Disabled Waiver Program.
• Supervising service providers to ensure compliance with the organization's health care plan for each member.
• Suggesting modifications to the health care plan as necessary to assist members in improving or stabilizing their health conditions.
• Coordinating information flow between members and service providers.
• Establishing and maintaining effective working relationships with all stakeholders.
• Helping members make informed choices by serving as their advocate regarding clinical status and treatment options.
• Contacting members via phone monthly and as required to assess their health conditions and needs.
• Conducting physical visits to members quarterly and as needed to evaluate their health conditions and needs, post COVID-19.
• Participating in weekly meetings aimed at discussing and resolving member issues and facilitating their needs.
• Complying with professional standards as specified by the protocols, rules, and regulations set forth by the organization and the Department of Community Health.
• Must possess a flexible work schedule to accommodate both scheduled and ad hoc meetings set by the organization.
• Bi-lingual skills are a plus, particularly in Vietnamese; knowledge of other languages such as Spanish, Korean, or Russian is also advantageous.
