Case Management Supervisor – Medicare/DSNP

This is a fully remote position, open to applicants in Washington.

📋 Description

• Provides leadership in both clinical and administrative aspects to care management teams.

• Responsible for overseeing the daily operations of both medical and behavioral health clinicians, as well as non-clinical personnel.

• Implements care management initiatives and ensures that program operations and documentation comply with regulatory standards.

• Monitors the performance and productivity of team members to guarantee adherence to guidelines and achievement of goals.

• Promotes collaboration with internal partners to ensure coordinated care.

⛳️ Requirements

• At least three (3) years of clinical experience in an acute care or public health environment.

• Minimum of three (3) years of experience as a case manager in an ACO, MCO, or BHO is required.

• Three (3) years of supervisory or leadership experience in a healthcare or managed care setting is preferred.

• Bachelor’s degree in nursing or a Master’s degree in social work and/or a related behavioral health field.

• A current, unrestricted RN license in the State of Washington OR a current, unrestricted LICSW license in the State of Washington.

• Must possess or obtain a Case Management Certification within the first two (2) years of employment.

• Proficient in Microsoft Office applications such as Outlook, Word, and Excel.
