This is a fully remote position, open to applicants in Ohio, +3 more states.
• Allocate 50-75% of your day to interacting with members and managing their care.
• Leverage available resources to engage with and support “hard-to-reach” members.
• Collaborate with members to create personalized care plans.
• Ensure accurate documentation of care management activities is maintained.
• Work alongside the Interdisciplinary Care Team to overcome barriers to care.
• Identify and link members with health plan benefits and community resources.
• Comply with regulatory requirements within designated timelines.
• Consult with the Care Manager RN for clinical insights and decision-making.
• Partner with the Care Manager RN to guarantee that members’ needs are met.
• A minimum of 2 years of experience in a health-related field.
• At least 2 years of experience in customer service.
• Proficient in Microsoft Office Suite (Word, Excel, Outlook, OneNote, Teams).
• Must have access to a private, dedicated space to perform work effectively.
• Dependents should have alternative care arrangements during work hours.
• Experience in providing care management for Medicare and/or Medicaid members is preferred.
• Familiarity with individuals facing SDoH needs, chronic medical conditions, and/or behavioral health challenges is preferred.
• Experience in conducting health-related assessments and facilitating care planning processes is preferred.
• Bilingual abilities, particularly in English and Spanish, are preferred.
