Case Management Social Worker

This is a fully remote position, open to applicants in Arizona, +2 more states.

📋 Description

• Proactively assist in the coordination of care, whether in person or remotely, across various environments (inpatient, outpatient, post-acute, ER, home care, etc.) while upholding strict confidentiality and the highest standards of professionalism.

• Engage actively with members and providers, both in person and remotely, who are facing challenging health issues, collaborating with them to create tailored care plans, and guiding them through the healthcare system to ensure an exceptional experience while enhancing health outcomes.

• Evaluate and address the Social Determinants of Health (SDOH) needs of our members, and inform them about available resources for community support programs.

• Participate in crisis intervention initiatives for members.

• Act as a member advocate, assisting with problem resolution and addressing any obstacles to care or adherence to the care plan.

• Communicate with members through in-person visits, phone calls, and/or secure messaging to provide education regarding health conditions, new medications, and procedures.

• Ensure compliance with all relevant laws and regulations.

• Perform other assigned duties as needed.

⛳️ Requirements

• A minimum of 2 years of experience in medical social work within a healthcare facility setting (e.g., hospital, skilled nursing facility, home health, hospice, etc.).

• At least 1 year of experience in care coordination and navigation.

• A Master's in Social Work (MSW) from a nationally accredited institution recognized by the Council on Social Work Education.
