This is a fully remote position, open to applicants in United States.
• Oversee and coordinate healthcare services to guarantee comprehensive, patient-focused care.
• Evaluate patients’ medical requirements and formulate personalized care plans.
• Advocate for patients’ rights, preferences, and their access to suitable services.
• Track patient progress, adherence to care plans, and the effectiveness of interventions.
• Modify care plans and interventions as needed.
• Accurately and promptly document assessments, care plans, and outcomes.
• Create reports on patient status, care outcomes, and resource usage.
• Educate patients about medications, treatment plans, and self-care strategies.
• Offer emotional support and counseling to patients and their families.
• Encourage preventive care and wellness through health education and lifestyle advice.
• Ensure compliance with URAC standards, CMSA Standards of Practice, relevant regulations, and Valenz policies.
• Collaborate with internal teams to discover potential savings in health plan coverage.
• Perform additional duties as assigned.
• Active, unrestricted Registered Nurse (RN) license in the state of residence.
• Compact License (NLC) accepted if residing in a compact state.
• Minimum of 3 years of nursing experience in an acute care environment.
• Ability to thrive in a deadline-oriented setting.
• Strong organizational skills and attention to detail.
• Critical thinking and sound judgment in complex scenarios.
• Appropriate escalation of issues as necessary.
• Excellent communication and interpersonal abilities.
• Collaborative approach to care coordination.
• A quiet workspace free from distractions.
• Reliable internet connection.
• Compliance with company security protocols, including VPNs, secure passwords, and approved devices/software.
• Must be based in the US and adhere to company policies such as HIPAA.
• Experience in case management is a plus.
• Bilingual in Spanish is an advantage.
• Relevant certifications, such as CCM or CCP, are a plus.
• Willingness to obtain CCM certification within 18 months of hire is a plus.
