This is a fully remote position, open to applicants in United States.
• Deliver skills-focused individual and group sessions to caregivers receiving treatment at Equip.
• Employ motivational techniques with caregivers to enhance their engagement with care plans, which includes overseeing adherence to care plans and fulfilling program components.
• Contribute to multidisciplinary treatment team meetings to facilitate patient progress in treatment.
• Actively engage in listening and guiding caregivers to foresee challenges and adopt coping strategies.
• Maintain an availability calendar for caregivers to arrange appointments.
• Perform proactive outreach (phone calls/messaging), recover missed appointments, provide reminders, and track support to ensure full adherence to the care plan.
• Engage in problem-solving to guarantee compliance with care plans, which includes identifying and offering practical solutions to treatment adherence obstacles.
• Ensure care plans are personalized by making program recommendations and coordinating services to meet caregiver needs.
• Assist caregivers in creating advocacy-related documentation, including letters for 504 Plan accommodation requests.
• Foster a relationship built on respect and trust with the caregiver.
• Complete timely documentation of caregiver interactions in accordance with Equip policies.
• Attend and participate in internal meetings as well as initial and ongoing training as assigned.
• Perform additional duties as assigned.
• A minimum of 2 years of experience working directly with patients in a protocolized care setting – including but not limited to coaching, case management, mentorship, social work, or milieu therapy.
• Demonstrated comfort in operating within structured care models or evidence-based protocols (e.g., adhering to specific clinical guidelines, behavior change frameworks, or care plans).
• Proven capability to balance structured protocols with deep empathy, meeting patients at their level while keeping them aligned with their care objectives.
• A solid track record of accountability for patient care plans, utilizing evidence-based frameworks to ensure patients remain on course and achieve clinical milestones.
• Experience in guiding patients and families toward achieving treatment goals while recognizing barriers and devising solutions to overcome them.
• Personal experience as a support for a loved one with an eating disorder in stable recovery.
• Ability to connect with a diverse range of individuals, each with different lived experiences.
• Ability to identify and assist patients and families in reaching their treatment goals.
• Passion for evidence-based care and a willingness to learn, practice, and develop new coaching techniques.
• Strong oral and written communication skills, with the ability to cultivate effective, professional, and collaborative relationships both internally and externally.
• Excellent organizational and time management skills, with a history of achieving goals.
• High attention to detail.
• Comfortable with utilizing and learning new technologies.
• Comfortable working remotely (including the necessity to be on camera).
• Capable of navigating ambiguity and thinking critically to address challenges in a dynamic, fast-paced environment.
• Availability to provide 26-33 hours per week of direct patient care, including new and follow-up sessions.
• Willingness to work two evenings until 8 PM of your choosing each week.
