Care Transitions Care Manager

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Oversee the continuous care for assigned individuals to ensure consistency and stability across various services and environments.

• Create and uphold detailed care plans that address medical, behavioral, and social requirements.

• Facilitate coordination of services among providers and community resources.

• Track progress towards care objectives and modify care plans in response to evolving needs.

• Involve individuals and families to promote ongoing participation in services and care plans.

• Deliver thorough care coordination for individuals transitioning between care environments while ensuring long-term stability.

⛳️ Requirements

• Must fulfill QP status via one of the specified qualification pathways: fully licensed by the NC governing board overseeing Human Services professions; or fully licensed RN authorized to practice in North Carolina with four years of full-time MH/DD/SA experience; or a master's degree in a human service field coupled with one year of full-time MH/DD/SA experience; or a bachelor's degree in a human service field with two years of full-time MH/DD/SA experience; or a bachelor's degree in another field with four years of full-time MH/DD/SA experience.

• Substance abuse professional alternative pathways necessitate the specified supervised alcoholism and drug abuse counseling experience.

• If assisting members with LTSS needs, a minimum of two years of previous LTSS and/or HCBS coordination, care delivery monitoring, and care management experience is required; this may overlap with QP experience.

• Must possess a valid driver's license.

• Must reside in North Carolina and be within 75 miles of Trillium's Southern Region.

• Eligible counties include: Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, and Scotland.

• Must be willing to travel within the catchment area as needed.

• All candidates are required to pass a drug test as a condition of employment.

• Preferred: Degree in Human Services.
