Care Specialist, LPN

This is a fully remote position, open to applicants in Oklahoma.

📋 Description

• Facilitates telephonic case management for intricate, high-risk members, encompassing the identification and evaluation of needs, care planning and coordination, as well as monitoring outcomes in alignment with GlobalHealth and departmental policies and procedures.

• Collaborates with providers when necessary to guarantee comprehensive, healthy, and advantageous outcomes.

• Interacts with sensitivity to family culture, ethnic background, race, language, gender, age, religion, socioeconomic status, sexual orientation, and mental and/or physical challenges.

• Through communication across departments, network interactions, and member outreach, the Case Manager stays informed of patient needs and condition changes, providing advocacy, support, quality care assessment, and crisis intervention.

• Coordinates services for members, including community resources and teamwork with other care team members.

• Instructs members and their caregivers on health conditions and self-management strategies.

• Engages in components of the SNP MOC, which includes completing HRA and collaborating with the member to achieve desired goals, under the guidance of the designated Case Manager.

• Executes TOC activities such as post-discharge assessment, medication reconciliation/review, and ensuring member access to follow-up care.

⛳️ Requirements

• Active Licensed Practical Nurse in the state of Oklahoma required.

• Preference for an active multi-state license.

• Previous experience in managed care or utilization management is preferred.

• Familiarity with current nursing processes, techniques, and established standards, including disease management, medications, and community resources.

• Strong attention to detail alongside excellent organizational and time management skills.

• Ability to communicate clearly and effectively, both orally and in writing.

• Proficiency in Microsoft software programs, including Word, Excel, and PowerPoint.

• Demonstrated capacity to work independently or collaboratively as part of a team.
