Care Plan Specialist

This is a fully remote position, open to applicants in Philippines.

📋 Description

• Perform patient intake procedures

• Document physician diagnoses and treatment plans

• Create and uphold customized care plans

• Conduct follow-ups after appointments

• Manage insurance processes, prior authorizations, and billing inquiries

• Ensure documentation is accurate, compliant, and current

• Deliver a smooth and supportive experience for every patient

⛳️ Requirements

• 2–3 years of experience in patient navigation, healthcare coordination, or medical records management

• Proficient understanding of medical terminology, diagnoses, and treatment plan documentation

• Experience in medical transcription—accurately recording physician explanations, diagnoses, and treatment plans

• Background in creating and maintaining personalized patient care plans and medical records in compliance with healthcare regulations

• Experience with insurance documentation, prior authorizations, and handling billing/claims inquiries

• Skilled in Google Workspace (Docs, Sheets, Drive), Slack, and Zoom

• C1+ level English (CEFR) for effective patient communication and coordination with US-based medical and administrative teams

• Pharmacy experience or knowledge of medication management (preferred)

• Practical experience with EMR/EHR systems (preferred)

• Experience in gathering and analyzing patient feedback to enhance the care experience (preferred)

• Familiarity with support and communication platforms such as Zendesk, Dialpad, Front, Intercom, or Airtable (preferred)
