This is a fully remote position, open to applicants in Illinois.
• Conduct phone consultations with patients participating in care management and/or remote patient monitoring programs, offering support and education regarding their chronic conditions.
• Inform patients about their health issues and empower them with lifestyle and behavioral strategies to effectively manage their chronic conditions.
• Aid patients in establishing and achieving objectives aligned with their provider-approved care plans.
• Keep precise and updated patient records, ensuring that all interactions and care plans are documented according to protocol.
• Address patient concerns and obstacles to care, striving to discover practical solutions that enhance patient adherence and outcomes.
• Deliver clear, compassionate, and effective communication to patients.
• Adhere to approved workflows when conveying patient needs to their healthcare providers.
• Engage in training sessions, team meetings, and quality improvement initiatives to refine the care navigation process and enhance the patient experience.
• Respond to remotely transmitted patient data, such as blood pressure, blood glucose, weight, and pulse oximetry, in accordance with established partner workflows.
• A current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN).
• Additional state licenses may be necessary and will be reimbursed by HealthSnap.
• Over 3 years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring.
• Strong communication and interpersonal skills.
• Excellent organizational and time management skills.
• Proficient in using electronic health records (EHR) and care management software.
• Capability to work independently as well as collaboratively within a team.
• Empathy and a patient-centered approach to care.
• Reliable internet connection and a HIPAA-compliant work environment, with proficiency in virtual communication tools (e.g., Zoom, Slack).
