Care Navigator

This is a fully remote position, open to applicants in United States.

📋 Description

• Respond to incoming calls, emails, and text messages from patients, caregivers, families, external providers, and other facilities.

• Schedule and maintain communication with patients via phone or email.

• Clarify health insurance benefits and eligibility for patients, processing copays and coinsurance as necessary.

• Under clinical supervision, pose screening questions and document patient safety, behavior, functionality, medications, care requirements, and advanced care plans.

• Under clinical oversight, inquire about caregiver challenges such as burden, depression, and coping difficulties.

• Relay screening information to the clinical team (nurse, social worker, doctor), who will guide the implementation of a care plan.

• Distribute educational materials and information regarding care with Isaac and other local community-based service options as indicated in the patient’s care plan.

• Conduct follow-ups after all appointments to provide next steps and coordinate care as documented by the provider.

• Request external medical records, laboratory results, and imaging outcomes.

• Foster a compassionate environment by offering emotional and psychological support to patients, caregivers, and families.

• Record patient care and instructions in the EHR.

• Coordinate patient care with external clinics, providers, and facilities as recorded in the EHR by the provider.

• Elevate patient concerns to the clinical team to ensure adherence to the appropriate scope of practice for this position.

• Safeguard participants’ privacy in accordance with HIPAA regulations.

• Previous experience in coordinating or navigating care for patients, caregivers, and their loved ones.

• Authentic empathy and patience.

• Outstanding time management and communication abilities.

• Bonus points for proficiency in Spanish.

• Experience with patient populations facing cognitive challenges.
