This is a fully remote position, open to applicants in United States.
• The Care Navigator Assistant plays a crucial role in providing virtual and administrative support to the Care Navigation team.
• This position is tasked with resolving administrative obstacles, overseeing patient panels, and tracking daily and weekly engagement objectives to prevent any patient from being overlooked.
• Proactively manage and clear the daily patient list, directing patients to their designated Care Navigators to guarantee 100% outreach to all panel members.
• Assist in monitoring the team's progress toward check-in targets and keep track of spreadsheets in collaboration with partner organizations.
• Review documentation logs to confirm that each patient interaction meets the required minimum of 20 minutes of billable time.
• Oversee the digital fax inbox, process incoming medical record requests, and conduct routine "chart housekeeping" within the EHR.
• Manage eligibility and benefits verification for patients within the panel.
• Distribute care plans, visit notes, and formal referrals as requested by Care Navigators.
• Immediately request neuroimaging reviews upon receipt of patient images and inform providers/Care Navigators when home safety assessments are finalized.
• Send appointment confirmations and reminders to enhance patient attendance rates.
• Research and identify local community resources for patients as directed by Care Navigators.
• Provide prompt administrative support for Care Navigators to ensure daily operations run efficiently.
• 1–3 years of experience in administrative or virtual assistant roles, preferably in a care management, neurology, brain health, or fast-paced medical clinic environment.
• Technical Proficiency: Familiarity with electronic health records (EHR), digital fax systems, and tracking spreadsheets (Excel/Google Sheets).
• Metric-Driven Approach: Comfortable operating in an environment with specific numerical objectives (outreach goals, tracking metrics, minimum billable minutes).
• Communication Skills: Outstanding written and verbal communication abilities; professional and empathetic when interacting with patients and coordinating with partner organizations.
