This is a fully remote position, open to applicants in New York.
• Supervising the Care Navigation program to ensure that existing services run smoothly while new programs are effectively implemented and integrated.
• Cultivating relationships with health plan partners and managing internal policies, procedures, workflows, and teams involved in care navigation efforts.
• Delivering telephonic care navigation services to individuals participating in various programs, including 1115 waiver programs, ILOS & VAB programs during the implementation and onboarding phases.
• Reporting directly to the Director of Intake.
• Occasional travel to offices in Iowa and New York may be necessary.
• Ensuring that care navigation services adhere to professional standards, organizational policies, and industry best practices.
• A Master's degree in Social Work.
• An active, unrestricted license as a Licensed Clinical Social Worker (LCSW), Licensed Independent Social Worker (LISW), Licensed Mental Health Counselor (LMHC), or an equivalent independent clinical social work license in the applicable state of residence.
• A minimum of two (2) years of experience in care management, care coordination, managed care, or a related healthcare environment.
• Experience working with individuals facing food insecurity, substance use disorders, behavioral health needs, homelessness, and/or transitioning from incarceration or institutional settings.
• Proven experience in developing processes, workflows, or programs.
• Previous supervisory or team leadership experience, especially in guiding teams through change or uncertainty.
• Capability to both capture and analyze data effectively.
• Exceptional written, verbal, and interpersonal communication skills.
• Strong organizational abilities with a capacity to manage multiple priorities at once.
• Proficiency in Microsoft Excel and other standard office technology platforms.
• Outstanding attention to detail and the ability to prioritize competing tasks and projects efficiently.
• Ability to work independently with minimal supervision while exercising sound professional judgment.
• Proficient in using standard office equipment and technology, including laptops and smartphones.
