Care Manager, Vietnamese-Speaking

This is a fully remote position, open to applicants in California.

📋 Description

• Oversee and analyze patient vital signs, assessments, and alerts within the Welby Health platform.

• Offer prompt, evidence-based recommendations and escalate findings of clinical significance.

• Accurately document patient interactions in compliance with internal protocols and regulatory requirements.

• Create, implement, and modify personalized care plans that cater to both clinical and social needs.

• Collaborate with physicians, specialists, and community resources to bridge gaps in care.

• Act as a liaison between patients and healthcare providers to enhance engagement and adherence.

• Provide patient education and coaching through phone calls and secure messaging.

• Encourage patients to manage chronic conditions and establish lasting health habits.

• Recognize barriers to care and address them with practical, patient-centered solutions.

⛳️ Requirements

• Valid and active registered nurse (RN) license in California.

• Approximately two (2) to five (5) years of direct patient care experience in medical-surgical or home care settings.

• Proficiency in both verbal and written Vietnamese.

• Bachelor’s degree in nursing or a closely related field in healthcare.

• Outstanding written and verbal communication abilities.

• Capability to work independently in a remote, technology-driven environment.

• Strong organizational skills with a keen attention to detail and follow-through.
