Care Manager, Telephonic Nurse

This is a fully remote position, open to applicants in South Carolina.

📋 Description

• Evaluates and assesses the needs and requirements of members to help them achieve and/or maintain an optimal state of wellness.

• Guides members and their families towards appropriate resources to enhance care and well-being.

• Utilizes a range of strategies, approaches, and techniques to address a member's physical, environmental, and psycho-social health challenges.

• Monitors patient care continuously through assessments and/or evaluations to ensure the patient is progressing towards their desired outcomes.

⛳️ Requirements

• A minimum of 3 years of clinical acute care experience.

• Valid Registered Nurse (RN) license in the State of South Carolina with no disciplinary actions.

• Extensive knowledge of Microsoft Office applications, including Word, Excel, and Outlook.
