This is a fully remote position, open to applicants in California, +3 more states.
• Assist patients who have recently been discharged from care or may require post-acute services.
• Guide individuals through healthcare options and post-acute care services.
• Utilize various tools to present patient choices and educational resources.
• Address customer inquiries, including location searches and complaints.
• Make determinations regarding the clinical level of care.
• Serve as a customer advocate throughout the referral process.
• Monitor and follow up on referral and admission results.
• Educate clients about Home Health, Hospice, and Palliative Care Services.
• An Associate's Degree is required.
• A Bachelor of Science in Nursing (BSN) is preferred.
• A compact state Registered Nurse (RN) license is mandatory.
• Minimum of 3 years of post-acute nursing experience is required.
• Experience in home health or hospice is preferred.
• Familiarity with home health, hospice, and palliative care services is essential.
• Current CPR certification is necessary.
• Proficiency in reading, writing, and speaking fluent English is required.
