β’ Assist patients who have recently been discharged from care or may require post-acute services.
β’ Guide individuals through healthcare options and post-acute care services.
β’ Utilize various tools to present patient choices and educational resources.
β’ Address customer inquiries, including location searches and complaints.
β’ Make determinations regarding the clinical level of care.
β’ Serve as a customer advocate throughout the referral process.
β’ Monitor and follow up on referral and admission results.
β’ Educate clients about Home Health, Hospice, and Palliative Care Services.
β’ An Associate's Degree is required.
β’ A Bachelor of Science in Nursing (BSN) is preferred.
β’ A compact state Registered Nurse (RN) license is mandatory.
β’ Minimum of 3 years of post-acute nursing experience is required.
β’ Experience in home health or hospice is preferred.
β’ Familiarity with home health, hospice, and palliative care services is essential.
β’ Current CPR certification is necessary.
β’ Proficiency in reading, writing, and speaking fluent English is required.
