
Care Manager, Telephonic Behavioral Health

This is a fully remote position, open to applicants in South Carolina.
• Evaluate and assess the needs of members in a telephonic setting
• Assist members and their families in achieving and maintaining wellness by facilitating access to resources
• Utilize various strategies and techniques to address a member's physical, environmental, and psychosocial health challenges
• Overcome obstacles that impede care delivery
• Monitor patient progress towards desired outcomes through ongoing assessments and evaluations
• Develop personalized care plans for members
• Support the Transition of Care (TOC) initiative
• Help with HEDIS completion follow-up visits after TOC and may perform field visits based on the needs of members and program specifications
• Must reside in the state of South Carolina
• Must hold an active license in South Carolina as one of the following: Licensed Professional Counselor (LPC), Licensed Independent Social Worker - Clinical Practice (LISW - CP), Licensed Masters Social Worker (LMSW), Licensed Baccalaureate Social Work (LBSW), Registered Nurse (RN), or Licensed Practical Nurse (LPN)
• At least 1 year of relevant work experience
• Willingness to travel up to 25%
