📋 Description

• Facilitating the coordination of care for SNP members

• Developing and sustaining a comprehensive care plan

• Overseeing care coordination with ICT team members

• Managing care transitions effectively

• Collaborating with fellow CM team members to assist beneficiaries during TOC events

• Engaging in case management and quality improvement committees

• Serving as a point of contact for the Interdisciplinary Care Team (ICT) alongside the PCP and beneficiary/caregiver

• Offering resources and benefits to help educate beneficiaries and caregivers in enhancing self-management skills and improving their quality of life

⛳️ Requirements

• Valid Registered Nurse license from the state where services will be delivered, or a current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC)

• A minimum of three (3) years of clinical experience in the healthcare field

• At least one (1) year of experience in Care Management, Case Management, or Population Health

• Bachelor's Degree in Nursing or an Associate of Science in Nursing (ASN) or a Nursing Diploma

• Experience managing Medicare, Medicaid, or SNP populations

• Minimum of one (1) year of experience in SNP Care Management
