Care Manager – RN

This is a fully remote position, open to applicants in Maryland.

📋 Description

• Accountable for the planning, development, execution, and monitoring of care plans for members.

• Overseeing the care management process for high-risk and special needs members.

• Conducting clinical and social evaluations in partnership with physicians.

• Building and maintaining professional relationships with service providers.

• Initiating and executing care plans to enhance the quality of care provided.

• Ensuring the accuracy and integrity of care management records and data.

⛳️ Requirements

• Must be a Registered Nurse with a valid state license.

• Minimum of 2 years of clinical nursing experience in either acute care or community settings.

• Proven experience assisting members through community agencies.

• At least 2 years of experience in care management.

• Familiarity with Managed Care Programs is preferred.

• A Bachelor of Science in Nursing (BSN) is preferred.

• Certified Case Manager (CCM) is preferred.
