Care Manager, RN

This is a fully remote position, open to applicants in New Hampshire.

📋 Description

• Responsible for developing, evaluating, and facilitating intricate care management activities primarily for members with physical health needs.

• Assesses member requirements, identifies barriers to obtaining the necessary care, and addresses social determinants of health.

• Creates continuous care plans/service plans and collaborates with healthcare providers.

• Recognizes issues/barriers to care and implements suitable care management interventions.

• Acts as a liaison between the member and the care provider team to guarantee appropriate care delivery.

• Conducts regular follow-ups and monitors the status of members.

• Offers resource support to both members and care managers.

• Gathers, documents, and maintains all relevant member information and care management activities.

• Provides and/or coordinates educational resources for members and their families or caregivers.

⛳️ Requirements

• Active NH RN Licensure is mandatory.

• Experience in both Physical Health and Behavioral Health is strongly preferred.

• A Degree from an Accredited School of Nursing is required.

• A Bachelor's degree in Nursing along with 2 to 4 years of relevant experience is necessary.
