Care Manager, RN

This is a fully remote position, open to applicants in Kansas.

📋 Description

• Assist the Physical Health Care Management team.

• Design, evaluate, and facilitate intricate care management activities for members primarily focused on physical health needs.

• Assess member requirements, obstacles to suitable care, and social determinants impacting health.

• Create continuous care plans and service strategies.

• Collaborate with providers, specialists, and community resources to meet unmet needs.

• Identify issues and barriers to care, providing appropriate interventions.

• Coordinate person-centered care among members, their families, caregivers, and care provider teams.

• Monitor and follow up on member status, condition changes, and progress towards care-plan objectives.

• Offer resource support for services related to employment, housing, independent living, justice, and foster care.

• Facilitate care management and ensure timely access to necessary care and services.

• Conduct outreach via telephone, digital platforms, home visits, and other locations.

• Collect, document, and maintain member information and care management activities to ensure regulatory compliance.

• Educate members and families/caregivers on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.

• Provide constructive feedback on opportunities to enhance care quality and cost-effectiveness.

• Carry out other assigned responsibilities and adhere to policies and standards.

⛳️ Requirements

• Degree from an accredited School of Nursing or a Bachelor's degree in Nursing.

• 2–4 years of relevant experience.

• Active Kansas Registered Nurse (RN) license is mandatory.

• State licensure and/or compact state licensure is required.

• Must reside in Kansas.

• Availability from Monday to Friday, 8:00 a.m. to 5:00 p.m.

• Approximately 10% travel is expected.

• Weekend coverage is not required.
