Care Manager, RN – Administrative Services

This is a fully remote position, open to applicants in Washington.

📋 Description

• Assess and oversee the evolving health and service requirements of patients.

• Deliver optimal personalized care to the patient/family unit utilizing the nursing process.

• Work in conjunction with the patient's Primary Care Physician and other healthcare team members to develop and execute a comprehensive written care plan.

⛳️ Requirements

• Must possess a Washington Registered Nurse License upon hiring.

• Bachelor's Degree in Nursing is preferred.

• A minimum of 3 years of recent experience in a healthcare setting, with a preference for home health case management experience.
