Care Manager – OB/GYN

This is a fully remote position, open to applicants in California.

📋 Description

• Oversee and analyze patient vitals, assessments, and alerts using Welby Health’s platform.

• Provide prompt, evidence-based recommendations and escalate significant clinical findings.

• Accurately document patient interactions in line with internal protocols and regulatory standards.

• Create, implement, and modify personalized care plans that cater to both clinical and social needs.

• Collaborate with physicians, specialists, and community resources to bridge gaps in care.

• Act as a liaison between patients and healthcare providers to enhance engagement and adherence.

• Conduct patient education and coaching through telephone and secure messaging.

• Empower patients to manage chronic conditions effectively and develop sustainable health habits.

• Recognize barriers to care and address them with practical, patient-centered solutions.

⛳️ Requirements

• Bachelor's degree in nursing or a related healthcare field.

• Active and valid registered nurse (RN) license in good standing in the state of Georgia.

• Approximately two (2) to five (5) years of direct patient care experience in Obstetrics and Gynecology (OB/GYN).

• Outstanding written and verbal communication skills.

• Capability to work independently in a remote, technology-driven environment.

• Strong organizational abilities with a keen attention to detail and follow-through.
