
Care Manager – Nursing

This is a fully remote position, open to applicants in Massachusetts.
• Ensure that all members receive timely care management (CM) throughout the continuum.
• Conduct telephonic outreach and/or face-to-face meetings using key motivational interviewing techniques to encourage program enrollment.
• Carry out departmental assessments and evaluate members comprehensively to identify their needs, health objectives, and obstacles to wellness.
• Create a tailored plan of care for each member, implement specific interventions from care managers, and update the care plan as necessary.
• Offer targeted health education, proactive strategies for managing conditions, and maintain communication with key providers and vendors involved in the member’s care.
• Work collaboratively with the member, caregiver, and facility care team to ensure a smooth transition to the next level of care.
• Act as a liaison with the interdisciplinary care team to enhance member outcomes.
• Pursue professional growth and development through self-directed learning activities.
• Registered Nurse with a current unrestricted license in the state of residence.
• May need to acquire additional state licensure in states where Point32Health operates.
• Comprehend and adhere to the provisions of state-specific Nurse Practice Act(s).
• National certification in Case Management is preferred.
• Bachelor’s degree or relevant equivalent experience (minimum required).
• A minimum of 5 years of relevant clinical experience.
• Proficiency in a second language is advantageous.
• Experience in home care or case management is preferred.
• Competence in Microsoft applications, technical concepts and principles, and computer software applications.
• Ability to collaborate effectively as a team member at various organizational levels.
• Expertise in assessment, planning, and managing member care.
• Advanced communication and interpersonal skills.
