Care Manager, LPN

This is a fully remote position, open to applicants in United States.

📋 Description

• Provide telephonic care management for patients enrolled in Curana's Value-Based Care Program.

• Analyze electronic health records (EHR) to pinpoint areas needing improvement in patient care.

• Act as a health coach to inform and support patients and their caregivers.

• Create individualized care plans focused on the needs of each patient.

• Aid in the closure of quality gaps through thorough clinical assessment.

• Arrange provider appointments for patients identified as at-risk.

• Relay updates on patient health status to Curana providers.

⛳️ Requirements

• Must possess an active, unrestricted compact LPN license.

• A minimum of 2 years of nursing experience is necessary.

• Experience in various care settings including inpatient, outpatient, or skilled nursing facilities.

• Proven experience in Case Management.

• CCM certification is highly preferred.

• Familiarity with Electronic Health Records is essential.
