Care Manager – LP

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Deliver proactive intervention and coordinate care for eligible Vaya Health members.

• Perform assessments and identify the needs of members.

• Collaborate with care teams and community partners to improve care processes.

• Ensure adherence to health regulations and standards.

⛳️ Requirements

• Master's degree in a discipline related to health, psychology, sociology, social work, nursing, or another relevant human services field.

• Minimum of 2 years of experience working directly with individuals who have behavioral health conditions.

• At least 2 years of prior experience in Long-term Services and Supports and/or Home Community Based Services coordination, care delivery monitoring, and care management.

• A valid license is required.
