Care Manager II

This is a fully remote position, open to applicants in Pennsylvania, +1 more state.

📋 Description

• Deliver remote operational assistance to Care Managers across various centers by filling in vacancies as necessary.

• Perform proactive outreach to patients to complete any outstanding care plans.

• Determine patient care needs by building personal connections with patients.

• Utilize patient data from dashboards and reports to spot trends and enhance patient engagement.

• Document patient care services accurately.

• Address the needs of patients related to opioid use disorder.

• Collaborate with patients to set individualized care goals.

• Organize and rearrange patient appointments.

• Monitor the clinical status of patients.

• Support physicians and clinicians during patient onboarding and orientation.

• Ensure the confidentiality and privacy of patient information.

⛳️ Requirements

• A minimum of a High School Diploma or GED is required.

• Experience in a fast-paced setting, servicing multiple clients, is preferred.

• Previous experience in healthcare, addiction medicine, community health, or care coordination is preferred.

• Familiarity with creating individualized care plans is a plus.

• Knowledge of compliant documentation practices is essential.

• Lived experience with addiction is advantageous.

• A proactive attitude and a willingness to think outside the box are essential.

• A mission-driven approach with a passion for assisting patients in leading better, healthier lives.

• Excellent organizational and time management skills with the capability to prioritize numerous tasks in a dynamic environment.

• Strong critical thinking and problem-solving abilities to identify care barriers and apply effective interventions.
