Care Manager – Foster Care

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Design, evaluate, and implement comprehensive care management initiatives primarily focused on mental and behavioral health needs.

• Assess member requirements through phone consultations or in-home visits and propose or facilitate care/service strategies.

• Conduct outreach through telephonic, digital, home, and/or other site visits.

• Create ongoing care plans for high-acuity members and identify relevant providers, specialists, and community resources.

• Coordinate efforts among members, families/caregivers, community resources, and care provider teams.

• Monitor care plans, member status, treatment side effects, complications, and clinical symptoms.

• Facilitate care coordination with providers and specialists to ensure timely access to services.

• Collect, record, and maintain member information and care management activities to ensure regulatory compliance.

• Educate members and families regarding procedures, provider instructions, treatment options, referrals, and healthcare benefits.

• Provide constructive feedback on opportunities to enhance care quality and cost-effectiveness.

• Adhere to all policies and standards and undertake additional assigned responsibilities.

⛳️ Requirements

• Master’s degree in Behavioral Health or Social Work, or a degree from an accredited nursing program.

• 2–4 years of relevant experience.

• Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN as per state contract specifications.

• Experience in behavioral health is necessary.

• Availability to work Monday–Friday, from 8am to 5pm.

• Applicants must be residents of the state of Missouri.

• Previous experience in foster care is preferred.
