Care Manager, Clinical - LPN

This is a fully remote position, open to applicants in Alabama.

📋 Description

• The LPN Care Manager assists patients through telephonic outreach, care coordination, and continuous health education.

• Duties encompass monitoring care plans, scheduling appointments, reviewing medical records, documenting in EHRs, and facilitating access to medical services.

• Manage and support the healthcare needs of patients via telephonic outreach.

• Conduct and document visits utilizing various technology platforms and EHRs.

• Monitor adherence to care plans, assess effectiveness, track patient progress promptly, and implement changes as required.

• Assist patients in accessing suitable medical and specialty providers.

• Educate patients and their family/caregiver(s) about relevant community resources.

• Coordinate the continuity of patient care with external healthcare organizations and facilities, including transitions from primary care to specialty care.

• Encourage patient self-management of diseases and behavioral modification interventions.

• Provide health counseling, education, and instruction to patients.

• Perform any additional duties necessary to promote our values, fulfill our mission, and adhere to our company principles.

⛳️ Requirements

• A valid New York LPN license is mandatory.

• Compact LPN license is preferred.

• Ongoing maintenance of licensure is a requirement for this position.

• A minimum of 2 years of experience in a healthcare environment dealing with patients with complex chronic diseases is preferred.

• Experience in Chronic Care Management is preferred.

• Proficiency in using a personal computer is essential.

• Technical and analytical skills are required.

• Strong communication, organizational, and interpersonal skills are essential.

• Familiarity with Microsoft Excel is preferred.
