Care Manager – BH, Foster Care

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Design, evaluate, and execute intricate care management tasks primarily focused on mental and behavioral health requirements.

• Deliver high-quality, cost-efficient healthcare results, tailored care plans, and educational support for members and their families.

• Assess member needs through phone consultations or in-home evaluations, and recommend or facilitate appropriate care or service plans.

• Perform outreach via telephone, digital platforms, home visits, and other locations as necessary.

• Create ongoing care strategies for members with high acuity levels.

• Identify and connect with providers, specialists, and community resources addressing mental health and substance use disorders.

• Coordinate efforts among members, families/caregivers, community resources, and care provider teams.

• Track care plans, member conditions, treatment side effects, complications, and clinical indicators.

• Support care coordination and ensure timely access to required services or care.

• Gather, record, and maintain member information and care management activities to ensure regulatory compliance.

• Educate members and their families about procedures, provider guidance, treatment alternatives, referrals, and healthcare benefits.

• Offer leadership insights on potential improvements in care quality and cost-effectiveness.

• Adhere to all policies and standards.

• Execute additional responsibilities as assigned.

⛳️ Requirements

• Master's degree in Behavioral Health or Social Work OR a degree from an Accredited School of Nursing.

• 2–4 years of relevant experience.

• Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN, as per state contract requirements.

• Required experience in behavioral health.

• Ability to work remotely from anywhere within the state of Missouri.

• Availability for work from Monday to Friday, 8am to 5pm.
