Care Manager, BH

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Approves and assesses the use of mental health and substance abuse services.

• Gathers and evaluates utilization data.

• Aids in discharge planning and care coordination.

• Offers support to members facing mental health and substance abuse challenges.

• Oversees inpatient and/or outpatient levels of care services.

• Conducts telephone triage, crisis intervention, and emergency authorizations.

• Creates discharge plans in collaboration with providers and facilities.

• Engages in quality improvement initiatives.

⛳️ Requirements

• Licensure is mandatory, specifically CEAP, LMSW, LCSW, LSW, LPC, or RN.

• At least 2 years of post-degree experience in healthcare, behavioral health, psychiatric, and/or substance abuse healthcare environments.

• Excellent organizational, time management, and communication abilities.

• Familiarity with utilization management processes, as well as mental health and substance abuse community resources and providers.

• Experience and knowledge in both inpatient and/or outpatient settings.

• Understanding of DSM V or the most up-to-date diagnostic edition.

• Capability to analyze specific utilization challenges, strategize, and implement solutions that positively affect quality of care.
