Care Manager – Behavioral Health

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Develops, evaluates, and facilitates intricate care management activities primarily focused on members with mental and behavioral health needs.

• Assesses the needs of the member through phone consultations or in-home visits.

• Suggests and/or coordinates the care plan/service plan to achieve optimal outcomes.

• Monitors the effectiveness of care plans/service plans and tracks member status and results.

• Provides leadership with insights on opportunities to enhance care quality and delivery for members.

⛳️ Requirements

• A Master's degree in Behavioral Health or Social Work, or a degree from an accredited nursing school is required.

• 2 to 4 years of relevant experience is necessary.

• Must possess a Licensed Master's Behavioral Health Professional credential (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN, as mandated by state contract requirements with behavioral health experience.
