This is a fully remote position, open to applicants in California.
• Oversee and analyze patient vital signs, assessments, and alerts utilizing Welby Health’s platform.
• Offer prompt, evidence-based recommendations and escalate significant clinical findings as necessary.
• Accurately and thoroughly document patient interactions in line with internal protocols and regulatory requirements.
• Create, implement, and modify personalized care plans that cater to both clinical and social needs.
• Collaborate with physicians, specialists, and community resources to bridge gaps in care.
• Act as a liaison between patients and healthcare providers to enhance engagement and adherence.
• Provide patient education and coaching through telephone and secure messaging channels.
• Empower patients to effectively manage chronic conditions and foster sustainable health habits.
• Recognize barriers to care and address them with practical, patient-focused solutions.
• Between two (2) to five (5) years of hands-on patient care experience in medical-surgical or home healthcare.
• Bachelor’s degree in nursing or a related healthcare discipline.
• Outstanding written and verbal communication abilities.
• Capability to work independently in a remote, technology-driven environment.
• Strong organizational skills with a keen attention to detail and follow-up.
