• Deliver proactive interventions and coordinate care for eligible members of Vaya Health.
• Identify and address inappropriate levels of care or gaps in services.
• Ensure that members receive the appropriate assessments and services.
• Collaborate with care teams to develop care plans and provide necessary support.
• A Master's degree in a relevant field such as health, psychology, sociology, social work, nursing, or another applicable human services discipline.
• A minimum of two years of experience working directly with individuals with behavioral health conditions.
• At least two years of prior experience in Long-term Services and Supports and/or Home Community Based Services coordination, care delivery monitoring, and care management.
• A valid license is required; acceptable licenses include Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), Licensed Clinical Mental Health Counselor (LCMHC), etc.
• Experience working directly with individuals with intellectual and developmental disabilities (I/DD) or traumatic brain injury (TBI) is preferred.
• Must reside in or near Alamance County, North Carolina.
• Assistance with clinical transition planning.
