Care Manager

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• The RCT (RB-BHT) Care Manager is tasked with delivering intricate care coordination and oversight for members primarily diagnosed with an Intellectual or Developmental Disability, which may also encompass secondary mental health or substance abuse issues, as well as complex medical or physical health needs.

• The RCT (RB-BHT) Care Manager facilitates integrated care management through multi-disciplinary collaboration and referrals, including but not limited to: support with the TCM opt-in/opt-out process, conducting care needs screenings, creating 90-day transition plans, completing/updates of CMCAs, care plan development, and service coordination as necessary and deemed appropriate.

• This role is primarily focused on members diagnosed with Autism Spectrum Disorder, especially children aged 18 months to 15 years.

• Employ person-centered planning techniques and strategies to collect information, understand the members being supported, and effectively address their specific needs.

• Provide education and guidance to members about their rights and responsibilities.

• Schedule and complete the NC Medicaid 1915i Assessment.

• Conduct Discovery activities (information gathering and assessment) as part of the planning process.

• Ensure that members and legally responsible persons are made aware of the services available (including all types of Medicaid and state-funded services), service options (e.g., Individual/Family Direction for Innovations participants), processes (e.g., requirements for specific services), and community resources.


⛳️ Requirements

• Must fulfill the Qualified Professional (QP) status.

• Fully licensed by the NC governing board that regulates Human Services professions; OR a fully licensed RN permitted to practice in North Carolina by the NC Board of Nursing, with four (4) years of full-time experience with the MH/DD/SA population served.

• OR have graduated from a college or university with a master’s degree in a human service field and possess one year of full-time, pre or post-graduate degree accumulated experience with the MH/DD/SA population, or be a substance abuse professional with one year of full-time, pre or post-graduate degree supervised experience in alcoholism and drug abuse counseling.

• OR have graduated from a college or university with a bachelor's degree in a human service field and have two years of full-time, pre or post-bachelor's degree accumulated experience with the MH/DD/SA population, or be a substance abuse professional with two years of full-time, pre or post-bachelor's degree supervised experience in alcoholism and drug abuse counseling.

• OR have graduated from a college or university with a bachelor's degree in a field outside of human services and accumulated four years of full-time, pre or post-bachelor's degree experience with the MH/DD/SA population, or be a substance abuse professional with four years of full-time, pre or post-bachelor's degree supervised experience in alcoholism and drug abuse counseling.

• Additionally, for those serving members with Long Term Services and Support (LTSS) needs, the care manager must meet the minimum QP qualifications defined above (A-D) and also have at least two (2) years of prior LTSS and/or Home and Community Based Service (HCBS) coordination, care delivery monitoring, and care management experience, which can be concurrent with the experience required to become a Qualified Professional.

• Must possess a valid driver’s license.

• Must reside within Trillium’s catchment area, which includes the following counties: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, or Wilson.

• Must be capable of traveling within the catchment area as required.
