This is a fully remote position, open to applicants in Washington.
• Designs, evaluates, and facilitates intricate care management initiatives primarily aimed at addressing mental and behavioral health needs of members.
• Ensures high-quality, cost-efficient healthcare outcomes by creating personalized care plans and providing education for members and their families regarding mental health and substance use disorders.
• Assesses member needs through phone consultations or in-home visits, considering available resources, and recommends and/or facilitates care/service plans for optimal outcomes.
• May engage in telephonic, digital, home, and/or on-site outreach to evaluate member needs and collaborate with available resources.
• Formulates ongoing care plans for members with significant acuity, identifying necessary providers, specialists, and community resources for effective care.
• Coordinates effectively among members and/or their families/caregivers, community resources, and the care provider team to ensure accessibility of identified services for members.
• Tracks care/service plans and/or member status and outcomes, observing changes in treatment side effects, complications, and clinical symptoms, and offers recommendations to adjust care/service plans based on identified member needs.
• Facilitates care coordination and collaborates with relevant providers or specialists to guarantee timely access to necessary care or services for the member.
• Gathers, documents, and maintains member information and care management activities to ensure compliance with current state, federal, and third-party payer regulations.
• Educates members and their families on procedures, healthcare provider instructions, treatment alternatives, referrals, and healthcare benefits.
• Provides insights to leadership regarding opportunities for enhancing care and quality delivery for members in a cost-effective manner.
• Performs additional duties as assigned and adheres to all policies and standards.
• A Master's degree in Behavioral Health or Social Work, or a degree from an accredited nursing program is required.
• 2 to 4 years of relevant experience is necessary.
• An active Washington (WA) Clinical License is required: LCSW, LMSW, LMFT, LMHC, LPC, or RN.
• Experience in managing or providing support for behavioral health and care services for foster youth aged 0–26 is essential.
• Capability to manage inbound and outbound calls, coordinate with hospitals, and assist in care transitions.
• Strong aptitude for supporting crisis interventions and navigating complex cases is needed.
