This is a fully remote position, open to applicants in Missouri.
• Designs, evaluates, and implements intricate care management activities primarily addressing mental and behavioral health needs of members.
• Assesses member needs through phone consultations or in-home visits concerning available resources.
• Creates continuous care plans for members exhibiting high levels of acuity.
• Facilitates care coordination and collaborates with relevant healthcare providers or specialists.
• Oversees care plans/service plans and/or monitors member status and outcomes for any alterations.
• Gathers, records, and safeguards member data and care management activities.
• Educates members and their families on procedures, healthcare provider instructions, treatment alternatives, referrals, and healthcare benefits.
• Provides insights to leadership on potential enhancements for care quality and delivery for members.
• A Master's degree in Behavioral Health or Social Work or a degree from an accredited nursing school is required.
• 2 to 4 years of relevant experience is necessary.
• Must hold a Licensed Master's Behavioral Health Professional credential (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN as per state contract requirements, with experience in behavioral health.
