This is a fully remote position, open to applicants in Illinois.
• Develop, evaluate, and implement intricate care management activities focusing mainly on mental and behavioral health requirements.
• Design individualized care plans and offer education to members and their families about mental health and substance use disorders.
• Assess member needs through phone consultations or in-home visits and suggest or facilitate care and service strategies.
• Conduct outreach via telephonic, digital, home visits, and other site engagements.
• Create continuous care plans for members with high acuity needs.
• Identify necessary providers, specialists, and community resources for effective care.
• Coordinate communication among members, families, caregivers, community resources, and care provider teams.
• Monitor care plans, treatment side effects, complications, clinical symptoms, and member outcomes.
• Ensure care coordination and prompt access to essential care and services.
• Collect, document, and uphold member information and care management activities to meet regulatory compliance.
• Educate members and their families on procedures, provider instructions, treatment options, referrals, and healthcare benefits.
• Offer leadership insights to enhance cost-effective care and quality service delivery.
• Execute other assigned tasks while adhering to organizational policies and standards.
• Candidates must reside in Illinois within one of the specified eligible counties.
• A Master's degree in Behavioral Health or Social Work, or a qualification from an accredited nursing school is required.
• 2–4 years of relevant experience is essential.
• For the Illinois YouthCare plan: a graduate degree and independent Illinois behavioral health clinician licensure, such as LCSW, LCPC, or PsyD, is necessary.
• An alternative qualification includes a bachelor's degree and Illinois RN licensure.
• Licensed Behavioral Health Clinician or a bachelor's-degreed RN with experience in behavioral health is required.
• Familiarity with the Child Welfare System or DCFS is advantageous.
