This is a fully remote position, open to applicants in United States.
• Assist in care management operations by engaging with patients and managing clinical and administrative workflows.
• Oversee electronic outreach dashboards and either send or reply to patient text messages.
• Reach out to low-acuity patients and escalate their responses to Nurse Care Managers as needed.
• Conduct wellness check-ins for patients suffering from chronic illnesses, including chronic kidney disease (CKD) and congestive heart failure (CHF).
• Facilitate new patient engagement and follow up with patients after graduation.
• Promote medication adherence by verifying prescription pick-up and addressing any barriers.
• Input summaries, notes, assessments, and various documentation into partner electronic medical records (EMRs).
• Conduct chart sweeps and either resolve or delegate triage and phone encounter tasks.
• Handle email referrals and assign them for outreach purposes.
• Manage post-acute admissions along with Skilled Nursing Facility (SNF) attribution requests.
• Generate admission, discharge, and transfer (ADT) reports and oversee patient cohort assignments.
• Distribute patient communications, including introductory letters, educational materials, and "Unable to Reach" letters.
• Complete administrative duties and engage in centralized or specialized team models.
• Work collaboratively with clinical teams, operations, and external practice partners to ensure a unified patient experience.
• Undertake other related tasks as assigned.
• High school diploma or equivalent is mandatory.
• An associate’s or bachelor’s degree in healthcare, health administration, or a related field is preferred.
• A Licensed Professional Nurse (LPN) license is preferred.
• A minimum of 2 years of experience in care coordination, healthcare operations, medical records, or patient outreach is required.
• Candidates must reside in the Pacific or Mountain time zones.
• Familiarity with chronic conditions such as CHF, CKD, diabetes, and hypertension is necessary.
• Understanding care management, transitions of care, and patient engagement workflows is essential.
• Experience with EMRs/EHRs and healthcare technology platforms, including Commure, CareInsights, and CRM tools, is required.
• Proficiency in Microsoft Office or Google Workspace and the ability to navigate multiple systems simultaneously is expected.
• Experience in handling inbound calls and using call center tools, such as RingCentral, is needed.
• Strong written and verbal communication skills, particularly in patient-facing messaging, are essential.
• Ability to engage with patients empathetically and professionally, especially those with complex needs, is required.
• Excellent collaboration skills with nurses, clinicians, and external partners are necessary.
• Capability to manage a high volume of task-oriented work amid competing priorities is essential.
• Comfort in dynamic, early-stage, or growth-oriented healthcare settings is needed.
• Ability to work independently while adhering to standardized workflows is a must.
