Care Management Clinician

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Support the attainment of client wellness and independence through advocacy, assessment, planning, communication, education, resource management, and service facilitation.

• Gather and evaluate patient information relevant to their history, condition, and functional capabilities.

• Create a detailed, personalized care management plan.

• Connect clients with suitable providers and resources across the spectrum of health and human services and care environments.

• Ensure that the care delivered is safe, effective, client-centered, timely, efficient, and equitable.

• Collaborate with other PacificSource staff to guarantee high-quality customer service is maintained.

• Serve as an internal resource by addressing inquiries that require medical or contractual interpretation.

⛳️ Requirements

• A minimum of three (3) years of clinical experience, including case management.

• Experience in the insurance industry is preferred.

• An active, unrestricted Registered Nurse (RN) license, Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Clinical Social Worker (LCSW) credential is required.

• Certified Case Manager Certification (CCM) must be obtained at the time of hire or within two years of the hire date.

• Familiarity with health insurance and state-mandated benefits.

• Proficient experience and knowledge in case management practice.

• Ability to effectively interact with individuals facing various health issues and concerns.

• Understanding of contractual benefits and options that exist outside of those benefits.

• Competence in utilizing computerized systems for data documentation and retrieval.

• Ensures the confidentiality, privacy, and security of patient health records.
