
Care Coordinator β Part Time

This is a fully remote position, open to applicants in Minnesota.
β’ Deliver care coordination services to individuals of all ages who have disabilities, mental health issues, or terminal illnesses.
β’ Employ person-centered practices to facilitate services for individuals with disabilities among various health and social service professionals and across different care settings.
β’ Listen-Learn-Lead: Exemplify the mission and values of Accord, empowering individuals to lead their best lives.
β’ Engage professionally with interdisciplinary team members, including the individuals supported, family caregivers, guardians, employers, case managers, community providers, and other relevant stakeholders.
β’ Evaluate physical and mental wellness, needs, preferences, and abilities; use this information to create customized treatment plans.
β’ Advocate for individuals, ensuring awareness of and sensitivity to culturally appropriate care.
β’ Deliver timely and effective services that enhance quality and cost-efficient health outcomes.
β’ Manage a dynamic caseload of 60-80 members.
β’ Attend all mandatory annual and additional meetings, showcasing a high level of professionalism and genuine concern for the individuals supported.
β’ Represent the agency to the individuals we assist, our community partners, and external stakeholders.
β’ Complete documentation for all services rendered in the designated electronic health record/database system within one business day of service delivery.
β’ Must reside in St. Louis County or nearby counties.
β’ Bachelor's or Masterβs degree in social work, with a license from the Minnesota Board of Social Work / OR a Registered Nurse license from the Minnesota Board of Nursing.
β’ A minimum of 2 years of professional experience in social services is required, ideally with older adults and/or individuals with disabilities.
β’ Familiarity with health, social services, and funding sources that assist individuals.
β’ Self-motivated with the ability to work independently as well as collaboratively within a team.
β’ Strong organizational skills and the capability to thrive in a fast-paced environment.
β’ Proficient in technology with a proven ability to manage multiple computer systems.
