This is a fully remote position, open to applicants in Ohio.
• Collaborates with families to identify care coordination needs and services in alignment with service delivery standards and program outcomes, ensuring optimal outcomes for children, youth, and families.
• Works alongside families to articulate cultural factors that impact strengths, functioning, and family interaction styles, promoting ongoing engagement and success in care planning.
• Recognizes the strengths of children, youth, and families for effective care coordination engagement and fostering healthy outcomes.
• Coordinates family-centered services for children, youth, and families within their home, school, and community settings.
• Collaborates with families to confirm that services outlined in care plans are the most suitable, least restrictive, and address the safety and treatment needs of the child, youth, and family.
• Engages and cultivates positive relationships with children, youth, and families in partnership with child and family teams to facilitate the successful integration of team members and care plans.
• Develops collaborative and innovative partnerships with community resources to address the diverse needs of youth and families.
• Maintains essential documentation, participates in program evaluations, attends team and program planning meetings, engages in cross-systems training, and stays informed about community resources.
• Keeps up to date with all training requirements, including but not limited to High Fidelity Wraparound, MI, Cultural Humility, and more.
• Performs all other duties as assigned.
• Experience providing services and/or support to children and families involved in behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field: three years with a high school diploma or equivalent; or two years with an associate degree or bachelor's degree; or one year with a master's degree or higher.
• Preferred knowledge and experience in Hi-Fidelity Wraparound (Certification provided upon employment).
• Two years of experience in a coordinated supportive services or care coordination role is preferred.
• Experience working with individuals with autism spectrum disorders and developmental disabilities is preferred.
• Experience in one or more of the following areas: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, child development.
• Must demonstrate cultural humility or responsiveness with the training and experience to manage complex cases.
• Possess the qualifications and experience necessary to work with children and families facing severe emotional disturbances (SED), trauma, co-occurring behavioral health disorders, and those engaged with one or more child-serving systems (e.g., child welfare, intellectual and developmental disabilities, juvenile justice, education).
• Excellent organizational skills with the ability to maintain focus and prioritize multiple tasks.
• Demonstrates a high level of cultural awareness.
• Experience working with multi-need individuals and families.
• Comprehensive knowledge of community service systems.
• Willingness to participate in and lead cross-systems care coordination.
• Ability to communicate effectively through verbal and written expression.
• Must be capable of functioning in an Internet-based, automated office environment.
• A valid Driver's License is required.
• Ability to operate, receive, and conduct video calls and/or meetings in an automated office environment is essential.
• A minimum of 100mbps for high-speed internet connection is required.
