This is a fully remote position, open to applicants in Florida.
β’ Act as the main contact for patients after they have been triaged by a Physical Therapist or Advanced Practice Provider.
β’ Assist patients in navigating the recommended next steps and provide support throughout their care journey.
β’ Perform proactive follow-up outreach to enhance adherence, re-engagement, and continuity of care.
β’ Offer a supportive, patient-focused experience that aligns with RightMove standards.
β’ Oversee care navigation and transitions between different levels of care in collaboration with Physical Therapists, APPs, and external referral sources.
β’ Execute and monitor referrals, including the confirmation and receipt of external documentation.
β’ Employ basic coaching techniques, such as goal setting, accountability check-ins, and problem-solving for barriers, to promote adherence.
β’ Collaborate closely with Physical Therapists, APPs, and other team members to ensure coordinated, patient-centered care.
β’ Raise clinical inquiries, changes in condition, or concerns to the appropriate provider when necessary.
β’ Work with leadership to assess, verify, and enhance care coordination workflows and policies.
β’ Accurately and consistently document patient interactions, follow-ups, outcomes, and care coordination activities in the electronic health record.
β’ High school diploma or GED (an associate degree or higher is preferred).
β’ Experience in healthcare coordination, patient support, coaching, or roles within a medical office.
β’ Comfort in conducting structured follow-up conversations that emphasize motivation, behavior change, and adherence, rather than just scheduling.
β’ Strong organizational abilities with dependable follow-through and the capability to function within structured, dynamic workflows.
β’ Excellent communication skills, both written and verbal.
β’ Ability to work efficiently in a remote setting and manage digital workflows.
β’ Preferred experience in telehealth, digital health, musculoskeletal care, or physical therapy environments.
β’ Proficiency in the Spanish language.
