This is a fully remote position, open to applicants in United States.
• Assist with the enrollment and onboarding processes for CCM patients.
• Confirm patient eligibility according to Evergreen and payer program standards.
• Ensure that patient consent forms are completed before outreach begins.
• Actively engage with patients on a regular basis.
• Identify patients who are inactive or unengaged and escalate issues as needed.
• Conduct text and phone outreach to enrolled CCM patients on a weekly basis.
• Meet the required engagement time standards in line with Medicare CCM billing regulations.
• Aid in achieving the necessary engagement time for billing compliance.
• Reinforce care plans directed by providers during patient interactions.
• Identify obstacles to adherence, such as issues with medication access, transportation, or social needs.
• Accurately document patient interactions in the CCM patient management platform and EHR systems.
• Monitor engagement time in accordance with Medicare CCM billing criteria.
• Maintain documentation that is ready for audit at all times.
• Ensure compliance with HIPAA and PHI regulations.
• Facilitate patient access to their referring providers and clinical staff.
• Assist with care transitions following hospital stays or emergency department visits.
• Coordinate referrals and subsequent appointments.
• Escalate any symptoms or medication concerns to licensed clinical personnel.
• Use CCM platforms and EHR systems on a daily basis.
• Oversee patient messaging workflows.
• Collaborate with remote clinical and operational teams.
• Certified Medical Assistant certification is required.
• 1–3 years of experience in the healthcare field.
• Experience in chronic disease management or nephrology is preferred.
• Excellent clinical assessment, communication, and documentation abilities.
• Comfortable using two systems (EHR + CCM platform) concurrently.
• Capability to manage a structured, metrics-driven workflow with dependability.
• Intermediate proficiency in MS Office Suite, including Outlook and Teams.
• Ability to effectively work in a predominantly remote environment:
• Home internet must support a minimum download speed of 25 Mbps and upload speed of 10 Mbps, with Cable, Fiber, or DSL connections hardwired to the internet device recommended.
• Evergreen will supply remote employees with telephony applications and equipment necessary for their roles.
