This is a fully remote position, open to applicants in Tennessee.
• Engage in outreach efforts to inspire, assist, and inform members about the advantages of various programs.
• Perform evaluations of members' statuses and collaborate with them to create a care plan that aligns with their objectives.
• Assess individual care requirements and relay medical information to healthcare professionals.
• Oversee a caseload of members to guarantee timely contact with each individual.
• Coordinate care with providers and arrange appointments as necessary.
• Encourage members to actively participate in their health and overall wellness.
• Recognize and assist in addressing needs related to Social Determinants of Health.
• Accurately and thoroughly document actions taken within a care management platform.
• Conduct a high volume of outreach to members, families, providers, or other necessary recipients.
• Possess an active and unrestricted Licensed Practical Nurse (LPN) or Registered Nurse (RN) license.
• Have 2+ years of experience as a care coordinator, case manager, community health worker, pharmacy technician, or social worker.
• Reside in Tennessee.
• Be fluent in English; fluency in Spanish is preferred.
• Maintain a dedicated home office for remote work.
• Have a minimum of 1 year of remote work experience.
• Experience working with Medicaid and/or Dual Eligible patients is required.
• Possess strong interpersonal, communication (both verbal and written), and problem-solving skills.
• Have experience interacting with customers via phone and text messaging.
• Familiarity with documenting case notes in a care management or electronic health record platform.
• Experience with motivational interviewing techniques.
