Care Coordinator, LPN, LVN, Day Shift

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee patient census utilizing a resolution-oriented strategy to address gaps in both clinical and non-clinical patient care.

• Identify and facilitate access to community resources that can enhance patient care.

• Deliver patient education and promote health literacy regarding the management of chronic conditions.

• Conduct medication management activities, which include recognizing potential medication issues, performing reconciliations, ensuring adherence, and coordinating refills.

• Help guarantee the prompt delivery of services to patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patient inquiries and foster open communication to better understand their requirements.

• Assist in managing referrals and scheduling appointments.

⛳️ Requirements

• Valid Compact/Multi-State license (LPN) (LVN)

• Proficient with Microsoft Office Suite

• Strong written and verbal communication skills

• Experience with Epic (Preferred)

• Bilingual capabilities (Preferred)

• Additional Single State licensures (Preferred)
