Care Coordinator, LPN, LVN

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Oversee patient census with a focus on resolving issues in both clinical and non-clinical patient care.

• Identify and connect patients with community resources that can enhance their care.

• Educate patients and promote health literacy regarding the management of chronic conditions.

• Conduct medication management, which includes recognizing potential medication issues, performing reconciliation, ensuring adherence, and coordinating refills.

• Support the timely provision of services to your patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patients' inquiries and foster an open dialogue to better understand their needs.

• Assist in managing referrals and scheduling appointments.

⛳️ Requirements

• Active Compact/Multi-State license (LPN) (LVN)

• Proficiency in Microsoft Office Suite

• Strong written and verbal communication abilities

• A minimum of 3 years of hands-on patient-facing experience

• Experience with Epic (Plusses)

• Bilingual capabilities (Plusses)

• Additional Single State licenses (Plusses)
