Care Coordinator, LPN, LVN

This is a fully remote position, open to applicants in Utah.

📋 Description

• Oversee patient census with a focus on resolution to address gaps in both clinical and non-clinical care for patients.

• Identify and connect patients with community resources that enhance their care experience.

• Deliver patient education and improve health literacy regarding the management of chronic illnesses.

• Conduct medication management, which includes identifying potential medication issues, performing reconciliations, ensuring adherence, and coordinating refills.

• Support the timely provision of services for patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patient inquiries and foster an open dialogue to better understand their needs.

• Assist with managing referrals and coordinating appointment scheduling.

⛳️ Requirements

• Current Compact/Multi-State license (LPN) (LVN)

• Proficient in Microsoft Office Suite

• Strong written and verbal communication skills

• Minimum of 3 years of experience in direct patient-facing roles

• Experience with Epic (preferred)

• Bilingual skills (preferred)
